Odontogenic infections are invariably caused by bacteria. An odontogenic infection is a dental problem that originates within a tooth or in the closely surrounding tissues. This infection is caused by a complex interplay of microflora that may normally inhabit the oral cavity. Odontogenic infections may remain localized to the region where they started, or spread into adjacent or distant areas. In addition to producing pain and discomfort, odontogenic infections can extend beyond natural barriers and result in potentially life-threatening complications, such as infections of the deep fascial spaces of the head and neck.

Odontogenic infections progress through 3 stages: inoculation, cellulitis and abscess If left untreated, mild infection and inflammation will progress in cellulitis, which is usually caused by aerobic bacteria. If it progresses further without resolution and/or treatment, an abscess, which is a pus-filled cavity.

Treatment of odontogenic infections includes diagnosis and management of the causative factor and, usually, prescription of appropriate antibiotics. It is imperative that the source of infection be addressed immediately. Placing a patient on antibiotics and rescheduling to have the source dealt with at a later time is not sound practice, as most often the infection will worsen. In addition, the patient’s medical status must be improved. The patient’s fluid and nutrition status should also be addressed, as many patients with odontogenic infections have decreased oral intake due to pain and difficulty in chewing or swallowing.

Odontogenic infections are among the most common human infections. Scientific evidence has linked severe infections with increased susceptibility to certain important systemic diseases and conditions such as cardiovascular disease, diabetes mellitus, adverse pregnancy outcomes, and pulmonary infections. Due to the association between odontogenic infections and other medical problems, it is imperative that dental infections be prevented when possible, or promptly recognized and adequately treated.
